She has only recently recovered from an extended bout of severe depression, and the remnants of it still often show in her tone and expression. She is fond of ghost stories and seems to enjoy freaking people out with them. She seems to like putting people on edge with her own demeanor as well. But, if confronted about it, she admits that she means no harm. Being a less than cheerful person, she gets her amusement in what ways she can, even if she ends up frightening others in the process. But if it's genuinely making someone uncomfortable, she'll stop if asked. She's not completely sullen of course - she gets joy from naturally happy and lively people, and from being able to help others in any way she can. Even if the opportunity is far less common and more minor than she had originally dreamed.
Since the earthquake in Kanehira City, Keiko has recovered from the worst of her depression, but is also a considerably more driven person. She has seen a bleak future where not just Kanehira City, but the entire world is destroyed by monsters - monsters that came from the hearts of humans. And now that she's seen it and returned, she is determined to stop that future at all costs.

It was only when another group of delinquents from a neighboring school picked a fight that Keiko woke up, so to speak, and realized what sort of path she was headed down. She ran from the situation and never looked back, cutting off contact with her former friends. In the following weeks, she tried to re-connect with her sister, re-focus on school, and try to catch back up to where she should have been. And while she got through high school and made it into Kanehira University with little grief, she still blames those days for what would happen later.
In university, Keiko had a rocky start, struggling to keep interest in her classes and nearly failing out of them in her first year. Meanwhile, she looked at her sister, doing well in her biology program and seeming to breeze through her own classes. The beginnings of depression started to set in as she struggled with her own insignificance, leaving her to do even poorer until something finally happened to wake her up. A detour through the factory district of the city led her back towards one of her former friends' old hangouts. There, she encountered one of them, now a factory worker straight out of high school. She found him seriously injured, claiming to have gotten into a fight and come out the worse for it. Keiko called for help and, in the meantime, did what first aid she knew to keep him in stable condition. After EMTs arrived, she learned that if she hadn't stepped in, he might have died or at least fallen into critical condition from blood loss before anyone else found him. Hearing that she'd effectively saved his life brought Keiko joy like she hadn't known in years.
From that day on, she decided she wanted to be able to help people, save people, like she had done then. And what better way to do it than to become a doctor? Hearing months after that her sister had successfully gotten into medical school left Keiko chasing after her once again, but this time she had her own desires and motivation. With that, her grades improved significantly, and while it was never the cakewalk she always imagined it had been for Miyoko, the remaining years of university seemed to breeze past. And yet, in the end, it wasn't enough. While her grades improved, her first year's poor performance dragged down her overall grades, and her test scores weren't enough to get her into med school.
On her parents' and sister's advice, Keiko instead entered a nursing program, and would come out as a registered nurse a few years later. But during that time her depression only began to grow in severity. She blamed fooling around in her high school days for her failure, beliving that if she'd continued to chase after her sister without looking ay anything else that she would have made it. Though she had become friends with the man she'd saved once again afterwards, she now cut off contact with him again, unable to bear seeing him and being reminded of that time. To make matters worse, the only place local she could get a job as a nurse was at the high school itself. Though she tried to act normal around the students, Keiko still came off as a very gloomy sort of person, and she would often completely come apart once she was back home. Then, when her depression was at its very worst, everything changed.
That day, in March of 2014, an earthquake struck the city. Keiko was in the business district at the time, desperately seeking something enjoyable to brighten her mood, when it hit, running right through the shopping district. Keiko wasn't sure what happened next, only that she blacked out, and when she next awoke, stood in a completely destroyed city. Clouds blocked out the sun, the air around her was far too cold for spring, and rather than earthquake damage, the remnants of the shopping district looked more like a war zone. Immediately, Keiko could be absolutely certain that an earthquake could not have done this. She explored for awhile, looking for survivors, but the only person she could find looked exactly like her...
An hour later, Keiko returned to the Kanehira City she knew. She looked back through a hole in the world, which closed behind her once she was clear of it. She came back believing she had seen the future, or at least one possibility of it. A future that could be accessed again if the 'rift' opened once more. But more importantly, a future that she couldn't allow to come to pass. She had a goal, a reason to still be in Kanehira City and working at the high school she had tried so hard to get away from. She had to save the world from the future she had seen, and the process of doing so would start right here in Kanehira.

Rather, she looks for those who know, like she does, what really lies on the other side of the rifts, and attempts to connect with them. Through them, and subsequent visits to the bleak future world, Keiko has put a name to the monsters from that side - Shadows, and theorized that they come from the hearts of humans. Treating the process by which this occurs as a disease and dubbing it "Kanehira Syndrome" in her notes, Keiko has started to look into a cause - and possible solutions.
